We are seeking a skilled Social Worker MSW to join our team at RWJBarnabas Health. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families.
Key Responsibilities:- Conduct comprehensive psycho-social assessments to identify patient needs and develop individualized care plans.
- Provide counseling and therapy services to patients and their families, addressing a range of issues including mental health, substance abuse, and social determinants of health.
- Collaborate with healthcare teams to develop and implement discharge plans, ensuring seamless transitions for patients.
- Build and maintain relationships with internal and external resources, including community agencies and entitlement programs, to support patient needs.
- Stay up-to-date with industry developments and best practices in social work, applying this knowledge to improve patient outcomes.
- Master's degree in Social Work (MSW) from an accredited program.
- New Jersey licensure (LSW or LCSW) required.
- At least one year of experience in an acute hospital setting, with consideration given to hospital internship experience.
