As an Advanced Practice Nurse (NP/ARNP) with YesCare, your responsibilities will include:
Delivering standard community care based on evidence-based practices for clinical services, including but not limited to sick calls, chronic care management, comprehensive assessments, and emergency care.
When necessary, conduct daily visits to the infirmary and document interactions in the patient's medical records as assigned. The objective is to ensure timely and appropriate care at the right location.
Provide on-call support as required for the position.
Adhere to the approved medication formulary before considering non-formulary procedures.
Document relevant observations and logical conclusions to justify the need for non-formulary medications on the request form for the Medical Director's approval when such medications are prescribed.
Utilize available in-house resources for treatment or resolution of identified issues before seeking off-site referrals. Provide emergency care on-site and respond appropriately in urgent or emergency situations.Demonstrate proper techniques for cardiopulmonary resuscitation and related pharmacotherapy with certification as mandated by credentialing.
Support medical care standards through compliance with existing policies and procedures. Act as a resource for other professionals or non-professional staff, providing guidance as necessary.
Attend required Medical Staff meetings. Offer monthly in-service training for staff as requested. Participate in monthly quality of care reviews and chart evaluations as needed. Mentor physician assistants or nurse practitioners as required.
Inform the Regional Medical Director and Site Health Services Administrator of any schedule changes. Assist in arranging coverage for medical services if unavailable for an extended duration.
Engage in all quality improvement initiatives, including sentinel event reviews.
Actively participate in the Utilization Review process and adhere to established procedures.
Deliver appropriate healthcare to patients.
Comply with state laws regarding the scope of practice.
Diagnose and treat patients within the designated scope of practice.
Order and, if necessary, conduct laboratory tests, X-rays, examinations, and electrocardiograms as appropriate.
Initiate consultation requests and non-formulary requests with supervisory physician's consent.
Educate patients regarding medical conditions, medication usage, dietary considerations, weight management, and the effects of prescribed treatments.
Perform additional duties and responsibilities as assigned by the supervisor.
