MDS Coordinator

The Resident Care Coordinator is a skilled healthcare professional responsible for ensuring accurate and up-to-date care plans and Minimum Data Sets (MDS) for all residents. This role requires a strong understanding of federal, state, and local regulations, as well as facility policies and procedures.
Key Responsibilities- Oversee the scheduling, completion, and accuracy of all MDS item sets.
- Coordinate the development of written care plans for each resident, ensuring they are resident-centered, realistic, and measurable.
- Chair resident care plan conferences and relay concerns from residents and their families to the Administrator.
- Monitor resident medical records for consistency and content, addressing discrepancies as needed.
- Participate in the development and implementation of the facility's Quality Assurance and Performance Improvement (QAPI) program.
- Current, unencumbered license to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in this state.
- Current CPR certification or ability to obtain within 90 days of employment.
- Working knowledge of OBRA, IDPH, and IDPA standards and methods of documenting.
- Ability to read, write, and speak English in an understandable manner.
- Leadership ability and willingness to work harmoniously with personnel.
- Ability to move intermittently throughout the workday.
- Ability to cope with the mental and emotional stress of the position.
- Ability to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people.
