Job Title: Physical Therapist - Float Position

We are seeking a skilled Physical Therapist to join our team in a float position. As a Physical Therapist, you will be responsible for providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The Physical Therapist will work in a dynamic and fast-paced environment, providing physical therapy services to patients with a variety of conditions. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Responsibilities:
  • Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
  • Establish a plan of care and implement individual treatment based on evaluation results and functional goals.
  • Monitor and evaluate outcomes and appropriately adjust the individualized treatment plan based on the patient's response to treatment and provide status updates to physicians.
  • Plan for and complete patient discharge, including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
  • Complete all required documentation following department policy, including patient progress notes, team conference reports, and discharge summaries as appropriate, to record patient care and progress of each patient assigned as well as daily statistics, billing, and scheduling.
  • Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly.
  • Perform various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
  • Demonstrate skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist.
  • Participate in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care.
  • Manage assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs.
  • Pursue professional growth through participation in continuing education courses, department in-services, and reading professional literature.
  • Orient new staff to clinical and department policies as requested.
  • Participate in the evaluation process of co-workers by providing verbal or written feedback as requested.
Requirements:
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.
  • Physical Therapist license issued by the state in which the team member practices (Wisconsin or Illinois).
  • Bachelor's Degree in Physical Therapy.
  • No experience required.
  • Intermediate computer skills.
  • Excellent communication and interpersonal skills.
  • Demonstrated organization, decision-making, and problem-solving skills.
