Hospice Nurse Job Summary

We are seeking a skilled and compassionate Hospice Nurse to join our team in the Plainview/Tulia/Hereford Area. As a Hospice Nurse, you will provide high-quality patient care and coordinate with the interdisciplinary team to ensure seamless care delivery.

Key Responsibilities:
  • Administer skilled nursing care to patients and families in their place of residence
  • Coordinate care with the interdisciplinary team, patient, and referring agency
  • Assume responsibility for coordination of care
Requirements:
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
  • Two years of experience as a Registered Nurse in a clinical care setting, with hospice experience preferred
Working Conditions:

Works in patient homes, nursing homes, assisted living facilities, hospitals, inpatient units, offices, and other environments in various conditions. May be exposed to blood and bodily fluids and infectious diseases. Ability to work flexible schedule and travel locally.
