As a Registered Nurse with our Hospice Team, you will play a vital role in providing exceptional patient care and support to individuals and their families.
**Key Responsibilities:**
* Conduct comprehensive assessments of patients and families to develop personalized care plans
* Collaborate with the interdisciplinary team to ensure seamless care coordination
* Administer medication and treatment as prescribed by the physician
* Provide emotional support and counseling to patients and their families
* Maintain accurate and detailed clinical notes
* Participate in interdisciplinary team meetings to discuss patient progress and needs
**Requirements:**
* Graduate of an accredited nursing program with current licensure
* 1 year of nursing experience, with 3+ years preferred
* Experience in hospice or a similar setting is a plus
* Strong knowledge of the hospice philosophy of care
* Commitment to clinical and documentation excellence
