Job Summary

We are seeking a highly skilled and compassionate Medical Social Worker to join our team at Amedisys. As a Medical Social Worker, you will play a critical role in assisting patients and families with health-related financial, social, and emotional concerns. You will work closely with our interdisciplinary team to provide comprehensive care and support to our patients.

Responsibilities
  • Assist patients and families with health-related financial, social, and emotional concerns
  • Provide social work services in accordance with the plan of care
  • Communicate changes in patient status to other healthcare team members and care center office staff
  • Educate patients and families on medical social work interventions and treatment plans
  • Perform care planning, transfer, and discharge functions
  • Identify risks or threats to patient health and implement appropriate interventions
Qualifications
  • Masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
  • One year of social work experience in a healthcare setting
  • Meets state and federal requirements for this position
  • Current CPR certification
