Requirements
- Master's degree in Social Work (MSW) or related field.
- Valid and current Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) license.
- At least 5 years of experience in clinical social work, preferably in home health or hospice settings.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations and healthcare teams.
