About the Role

We are seeking an experienced Clinical Care Coordinator to join our team as a Registered Nurse. In this role, you will be responsible for coordinating patient care, working closely with healthcare teams to ensure the best possible outcomes.

Job Responsibilities

  • Coordinate patient care activities, including scheduling appointments and procedures
  • Communicate with patients, families, and healthcare teams to ensure seamless care coordination
  • Develop and implement individualized patient care plans
  • Monitor patient progress and communicate with healthcare teams
  • Collaborate with healthcare professionals to ensure quality patient care

Requirements

To be successful in this role, you will need:

  • A current Registered Nurse (RN) license for the state in which you practice
  • One year prior experience as a Registered Nurse
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Benefits

We offer a competitive salary range of $68,000 - $88,000 per year, depending on experience, plus benefits including health, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
