- Assist with daily living activities, such as feeding, toileting, and ambulation.
- Monitor and report changes in patient condition to the hospice nurse or Case Manager.
- Participate in quality assessment and performance improvement activities.
- Collaborate with the hospice team to ensure seamless care delivery.
- Current CNA certification in the state of Pennsylvania.
- Minimum of six months of experience as a CNA, preferably in hospice care.
- Ability to read and follow written instructions and document care given.
- Strong communication and interpersonal skills.
- Ability to work with minimal supervision and maintain confidentiality.
