Job Summary:

We are seeking an experienced Hospice Registered Nurse/RN Case Manager to join our team in providing high-quality, personalized care to our patients. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare providers to develop and implement comprehensive care plans that meet each individual's unique needs.

Main Responsibilities:

  • Provide direct patient care services, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Conduct thorough assessments of patients' physical, emotional, and spiritual needs, identifying areas where support is needed.
  • Develop and implement personalized care plans, working collaboratively with patients, families, and healthcare teams to ensure seamless delivery of care.

Requirements:
