Key Responsibilities

As a dedicated Hospice Nurse at UnityPoint at Home, you will be instrumental in creating personalized care plans that enhance comfort and elevate the quality of life for patients during their final stages. Collaborating closely with patients and their families, you will be an essential member of a clinical team that provides expert support in end-of-life care. The Hospice RN applies advanced nursing knowledge, performs thorough assessments, and utilizes teaching and procedural skills to ensure high-quality patient care in their residences. You will contribute to the hospice care plan's development and execute patient care as directed by the physician, adhering to clinical best practices to achieve superior outcomes and an exceptional patient experience.

Qualifications

Essential Qualifications:

  • Graduation from a State Board approved Registered Nursing program and possession of a valid RN license in the relevant state(s).
  • A minimum of one year of nursing experience with a similar patient demographic.
  • Possession of a valid driver's license and automobile insurance as per state and organizational standards.
  • Completion of Mandatory Reporter training for Child & Dependent Adult Abuse.
  • Successful completion of the Person Centered Care (PCC) course within the first year of employment, along with annual competency validation.
  • Current Basic Life Support (BLS) Healthcare Provider certification.
  • Basic understanding of commonly used medications, including their administration, effects, and side effects.
  • Demonstrated competency in various nursing skills, with the ability to perform independently (including NPWT, complex wound care, IV therapy, catheter care, ostomy care, and chronic disease management).

Knowledge, Skills, and Abilities:

  • Familiarity with current medications and their administration methods.
  • Proficiency in a range of nursing skills, with the ability to work autonomously.
  • Excellent interpersonal skills.
  • Ability to function effectively as part of a collaborative team.
  • Understanding and application of relevant guidelines, policies, and procedures.
  • Basic proficiency in Microsoft Office applications.
  • Reliable transportation and the ability to drive safely in all weather conditions.
