Responsibilities:

  • Provide physical therapy services to patients in their homes, including evaluations, treatments, and progress assessments.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Conduct home safety assessments and provide education to patients and their families on safe mobility and transfer techniques.
  • Document patient progress and maintain accurate records.

Requirements:

  • NC is part of the PT compact privilege states
  • 1 year experience
  • Home Health experience
  • Successful completion of a physical therapy program at a school of physical therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association
  • BLSEMR: HomeCare HomeBase, Oasis, Outlook
  • 2-3 years of physical therapy experience preferred
