Job Summary:

Responsibilities:

  • Provide high-quality patient care to geriatric patients in a variety of settings, including hospitals, clinics, and long-term care facilities.
  • Conduct thorough assessments, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless patient care.
  • Collaborate with physicians, nurses, and other healthcare professionals to provide comprehensive care and support to patients and their families.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in geriatric care to provide evidence-based care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.

Requirements:

  • Current licensure as a Nurse Practitioner in Pennsylvania.
  • Board certification in Gerontology or a related field.
  • Minimum 2 years of experience as a Geriatrics Nurse Practitioner in a hospital or long-term care setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
