Job Description

The Registered Nurse (RN) in Home Health plays a critical role in our organization, working directly with patients to assess their needs, develop personalized care plans, and provide ongoing support and guidance throughout their treatment journey. This is a challenging yet rewarding opportunity for a skilled and dedicated nurse who is passionate about delivering exceptional patient care.

Key Responsibilities
  • Develops and implements comprehensive plans of care in collaboration with physicians and other healthcare professionals.
  • Conducts regular assessments to identify patient needs and updates care plans accordingly.
  • Provides education and support to patients and their families on disease management, self-care, and medication adherence.
  • Collaborates with interdisciplinary teams to ensure seamless coordination of care.
Requirements

To succeed in this role, applicants must possess the following qualifications:

  • Current RN licensure in state of practice
  • Current CPR certification
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
