Key Responsibilities
  • Conduct thorough assessments of patients' physical, social, and emotional needs, developing personalized care plans that meet their unique requirements.
  • Collaborate with physicians, nurse practitioners, and other healthcare professionals to coordinate patient care, making referrals as needed to optimize treatment outcomes.
  • Provide education and support to patients and families, empowering them to manage their health conditions effectively and promoting self-care techniques and prevention strategies.
  • Develop and maintain effective relationships with patients, families, and healthcare teams, ensuring open communication and collaborative problem-solving.
  • Manage complex caseloads, prioritizing tasks and allocating resources efficiently to meet patient needs.
Qualifications
  • Bachelor's Degree in Nursing from an accredited program, or equivalent experience.
  • Current registration to practice as a Registered Professional Nurse in New York State required.
  • Minimum one year of clinical nursing experience, including hospice, home care, or medical-surgical settings.
  • Demonstrated expertise in gerontological nursing, palliative care, or oncology desirable.
