About the Role:
Kaiser Permanente is seeking a highly skilled Medical Social Worker III to join our team. As a Medical Social Worker III, you will play a critical role in providing assessments, coordination, and implementation/evaluation of professional social services to patients and their families.
Key Responsibilities:
- Assessment and Intervention: Conduct bio-psychosocial assessments with patients, families, and significant others to identify social, emotional, and economic difficulties related to medical problems or which predispose to illness.
- Treatment Planning: Develop and implement treatment plans in accordance with departmental and medical center policies, ensuring quality services and adherence to professional standards.
- Crisis Intervention: Provide crisis intervention/treatment to adults, children, and families in emergent situations, including assessment, counseling, information/referral, and consultation to physicians and healthcare teams.
- Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, interventions, and outcomes, as well as regulatory data reporting and process coordination.
- Interdisciplinary Collaboration: Function as an effective team member, consultant, and liaison to medical, nursing, ancillary staff, and community agencies, promoting an interdisciplinary approach and competent follow-up and communication of accurate information.
Requirements:
- Education: Master's Degree in Social Work from a school accredited by the Counsel of Social Work Education.
- License: Licensed Clinical Social Worker (California).
- Experience: Minimum one (1) year experience working as a MSW in medical acute care setting or as part of a medical team, with MSW internships considered as part of experience.
- Additional Requirements: Current CA Drivers License, clear driving record for the past two (2) years, and proof of automobile insurance as required by law.
