Key Responsibilities:

  2. Collaborate with the multidisciplinary health team to develop and implement individualized treatment plans.
  3. Travel to off-site locations to provide social services and support to patients in need.
  4. Stay up-to-date with relevant laws, regulations, and best practices in social work and healthcare.
