Private Duty Nurse

United Nursing Solutions Inc is seeking a skilled and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide high-quality private duty nursing care to a medically complex patient in their home setting.
Key Responsibilities:- Comprehensive Nursing Assessments: Perform thorough nursing assessments to identify the patient's unique needs and develop individualized care plans.
- Tracheostomy Care: Provide expert tracheostomy care, including suctioning, stoma care, and tracheostomy tube changes as needed.
- G-Tube Management: Manage and maintain the patient's G-tube, including formula preparation, feeding, medication administration, and stoma care.
- Ventilator Support: Operate, troubleshoot, and maintain ventilator equipment, ensuring proper settings and patient response.
- Medication Administration: Administer medications via various routes (oral, enteral, parenteral) and monitor for therapeutic effectiveness and potential side effects.
- Monitoring and Documentation: Monitor and record the patient's vital signs, respiratory status, neurological function, and other relevant clinical data.
- Care Coordination: Collaborate with the patient's multidisciplinary healthcare team to ensure seamless care coordination.
- Education and Training: Educate and train family members and caregivers on proper care techniques, equipment use, and emergency response protocols.
- Quality Improvement: Participate in quality improvement initiatives, case conferences, and professional development opportunities to enhance patient care and nursing practice.
Requirements:
- Current, unencumbered LPN or RN license in the state of practice.
- Minimum of 3 years of clinical experience, with at least 2 years in a home health, private duty, or critical care setting.
- Demonstrated expertise in tracheostomy care, G-tube management, ventilator support, and other complex nursing interventions.
- Strong critical thinking, problem-solving, and decision-making skills.
- Excellent verbal and written communication skills, with the ability to effectively interact with patients, families, and healthcare professionals.
- Proficient in using electronic medical records and documentation systems.
- Current certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
- Ability to work independently, prioritize tasks, and manage multiple responsibilities in a dynamic home care environment.
- Flexibility to work various shifts, including nights, weekends, and holidays, to ensure continuity of care.
