QUALIFICATIONS:

- A minimum of one year of verifiable experience in home care, private-duty care, community health, or clinical nursing rotations.
- Alternatively, one year of documented experience in a different field along with completion of an approved Home Health Aide Training course, if applicable.
- Effective communication skills in English or the primary language of the clientele.
- Successful completion of the Spring Hills Home Care Services Competency Evaluation.
- Proven ability to perform all required tasks competently.
- Clear criminal background check.
- Fulfillment of all medical and state requirements.
- Reliable transportation and communication methods.
- Punctuality, dependability, and sound judgment are essential.

JOB RESPONSIBILITIES:
- Adhere to the established Plan of Care.
- Assist clients with Activities of Daily Living (ADLs), which encompass tasks for self-care performed independently or with supervision, including mobility, grooming, bathing, dressing, eating, and toileting.
- Support clients with Independent Activities of Daily Living such as light housekeeping, shopping, laundry, and meal preparation as outlined in the Plan of Care.
- Accompany clients to appointments, noting that transportation is the client's responsibility unless otherwise agreed upon.
- Provide assistance with self-administered medications and reminders as per the Plan of Care.
- Maintain infection control and safety protocols.
- Report any changes in the client's condition to the Care Manager.
- Accurately document care provided and complete Activity Records.
- Execute all tasks as directed by the Care Manager or Business Office Manager (BOM).
- Foster positive relationships with clients and their families.
- Ensure high levels of client satisfaction.
- Meet all health screening deadlines.
- Encourage client independence to support rehabilitation efforts.
- Uphold client rights and confidentiality.

PERFORMANCE EVALUATION:
- Job performance will be assessed 90 days after employment and annually by the Care Manager or BOM, incorporating feedback from clients and coordinators.

EXPOSURE RISK:
- Potential exposure to blood or blood-borne pathogens; OSHA training will be required.
