Key Responsibilities:
  • Deliver care in both individual and group settings
  • Collaborate with primary care providers across the organization to offer education and monitoring services for all referred patients
  • Work closely with staff and providers to identify patients needing education and monitoring services
  • Independently manage a caseload of patients
  • Evaluate patients for their educational needs, follow up appropriately, and document in the electronic medical record

  • Utilize the established curriculum for patient care
  • Ensure professional and courteous care for patients

  • Provide self-management education during both group and individual visits
  • Instruct on the administration of subcutaneous and non-insulin injectable antihyperglycemic agents
  • Adhere to health and safety regulations and comply with regulatory agencies
  • Follow established departmental policies, procedures, and objectives
  • Enhance professional growth through educational programs and seminars
  • Attend various meetings, conferences, and seminars as required or directed
  • Perform other related duties as assigned by management

Required Qualifications:
  • Graduate of an accredited School of Nursing
  • Active and unrestricted Registered Nurse (RN) license in New York
  • Basic Life Support (BLS) certification
  • Minimum of 1 year of RN experience
  • At least 1 year of hospital experience
  • Experience with IV therapy and injectables
  • Proficiency in various software applications including Microsoft Excel, Word, Outlook, etc.
  • Familiarity with electronic medical records (EPIC preferred)
Preferred Qualifications:
  • Bachelor or Master of Science Degree in Nursing (BSN, MSN)
  • Certified diabetes RN educator
  • 3+ years of nursing experience
  • Experience with Epic EMR
  • Background in a medical office setting
  • Endocrinology experience
