Job Description

We are seeking a full-time Hospice Social Worker who would like to join our hospice team.

This role offers the opportunity to make a meaningful difference in the lives of patients and their families as they navigate the end-of-life process. You will work closely with our interdisciplinary team to create and implement a holistic plan of care that addresses the physical, emotional, and spiritual needs of each patient.

Fundraising for our End of Life Care Team

  • Hospice Social Worker, Monday-Friday, Dayshift
  • Create your own visit schedule and have work/life balance.
  • You will provide patients and families education and support throughout the end-of-life process.
  • You will work closely with the hospice interdisciplinary team to create and implement a holistic plan of care congruent with the patient's and family's wishes.
  • Focus on comfort, support, and quality of life.
  • Competitive Compensation Package
  • Starting at $25/hour, with guaranteed minimum of 40 hours paid per week, even if you work less.
