SunCrest Companion Services of Smithville is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality care to patients in their homes. As a LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:
  • Provide direct nursing care to patients in their homes, as prescribed by physicians.
  • Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans.
  • Ensure patient confidentiality and maintain accurate records.
Requirements:
  • Current LPN licensure in the state of practice.
  • Minimum one year of acute care experience, with previous home care experience preferred.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR certification.
