The ideal candidate for this position will possess a strong passion for working with individuals with developmental disabilities and have excellent communication skills. A Bachelor's Degree and minimum of one year of experience in the field is required.

Responsibilities:

  • Develop personalized support plans for individuals with developmental disabilities
  • Connect clients with approved Medicaid support providers
  • Monitor services to ensure effectiveness

Requirements:

  • Bachelor's Degree
  • Minimum of 1-year experience working with I/DD community
  • Strong written and verbal communication skills
  • Proficiency in using online web interfaces
  • Excellent time management and follow-up skills
