Job Opportunity

Baptist Home Health Care by BAYADA is seeking a compassionate and dedicated Physical Therapist to provide high-quality care to our adult and geriatric clients in Nassau and Northern Jacksonville, FL.

Responsibilities:

  • Make home visits to clients in designated territories to assess and develop individualized care plans.
  • Perform diagnostic tests and measurements to evaluate client mobility, strength, and balance.
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to promote client independence and safety.
  • Educate clients and their families on rehabilitative care and activities to support their health and well-being.
  • Accurately document client care management and services provided using electronic medical records.

Qualifications:

  • A current FL Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and prior experience with electronic medical records preferred.
