POSITION SUMMARY
We are seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for coordinating patient care and developing care plans to ensure optimal outcomes.
POSITION DUTIES
Key responsibilities include:
- Care Coordination: Collaborate with healthcare teams to develop and implement patient care plans.
- Continuum of Care: Ensure seamless transitions between healthcare settings.
- Patient Focus: Prioritize patient needs and preferences in care planning.
PREFERRED QUALIFICATIONS
Requirements include:
- RN License
- BLS Certification
- Acute Care Experience
LENGTH OF ASSIGNMENT
This is a 13-week assignment.
SHIFT / HOURS PER WEEK
