About the Position:
We invite applications for a Social Services Liaison to join our Inpatient Case Management Team. As a key member of our team, you will work closely with patients, families, and healthcare professionals to ensure a smooth transition from hospital care to post-acute settings.
Responsibilities:
- Participate in end-of-life matters and meet CMS Condition of Participation Requirements.
- Collaborate with case managers and multidisciplinary teams to identify patient needs and develop effective care plans.
- Educate patients and families on available post-acute options and address concerns related to choice.
- Function as a lead in assisting patients and families to access financial and other resources.
- Provide crisis intervention and support services as needed.
Requirements:
- Masters degree in Social Work required.
- Current licensure as a Licensed Independent Social Worker (LISW), Licensed Clinical Social Worker (LCSW), or Licensed Medical Social Worker (LMSW) from the State of New Mexico.
- A minimum of 10 years' experience in hospital social work or discharge planning required.
