We are seeking a skilled LPN to join our team at HCR Home Care in Eastern Monroe County. As a Licensed Practical Nurse, you will provide high-quality patient care in the comfort of their own homes.

Job Summary:

The LPN will administer practical nursing care to patients in accordance with physician's orders under the direction and supervision of a Registered Nurse. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Provide health education to patients and their families, as established by the RN.
  • Administer routine LPN nursing care, as outlined in the nursing care plan.
  • Observe and report patient reactions to treatments, including changes in physical or emotional condition.
  • Document patient services, changes in condition, and communication with RN Case Manager and physician.
Requirements:
  • Associate's degree in Nursing required.
  • Diploma as a Licensed Practical Nurse from an accredited program.
  • Current, valid LPN license issued from NYS Department of Education.
  • Experience in home health care preferred.
Work Environment:

The LPN will work primarily in a non-office setting and may be exposed to outdoor conditions. This role requires the ability to lift, carry, push, pull, or otherwise move objects, including the human body, up to 50 pounds occasionally.

Physical Requirements:

The LPN will be expected to stand, sit, hear, walk, talk, stoop, kneel, and perform repetitive motion on a daily basis.
