About the Role:
This full-time position requires a high level of professionalism, empathy, and attention to detail. You will work closely with caregivers, facilities, and community resources to ensure seamless care for our patients. Your primary responsibilities will include:
- Initiating contact and collaborating with various stakeholders to determine their capacity for placement of mental health patients
- Ensuring accurate and up-to-date information about care facilities and community resources
- Coordinating the assignment of new patients to qualified mental health professionals
- Preparing faxes, managing workflows, and performing administrative duties
Requirements:
To be successful in this role, you should possess a High School Diploma or GED, and preferably an Associate's Degree. We also require 2-5 years of experience in behavioral health, with a strong focus on client-centered care and excellent communication skills. If you have supervisor experience, that's a plus
