Key Responsibilities- Provide direct patient care and coordinate care with other healthcare professionals to ensure comprehensive care for our veterans.
- Assess, plan, implement, and evaluate care based on age-specific components and individual patient needs.
- Administer medications and procedures per established policies and guidelines.
- Collaborate with the interdisciplinary team to influence care outcomes and improve patient satisfaction.
- Manage assigned patient panels and ensure timely access to care.
- Provide health promotion, education, and coaching on wellness, disease prevention, and chronic care management.
- Participate in the development, implementation, and evaluation of policies, procedures, and guidelines.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
- At least one year of specialized nursing experience equivalent to the next lower grade level.
- 1-2 years of experience in Primary Care, Outpatient, or Med-Surg.
The Nurse (PACT-Outpatient) will work in a fast-paced, dynamic environment, providing care to a diverse population of veterans. The ideal candidate will be able to work effectively in a team-based setting, communicate effectively with patients and colleagues, and demonstrate a commitment to delivering high-quality patient care.
