Job Summary

The Department of Veterans Affairs is seeking a highly skilled and compassionate Nurse to join our team in the Primary Care Department. As a Nurse (PACT-Outpatient), you will be responsible for providing high-quality patient care and coordination to our nation's veterans.

Key Responsibilities
  • Provide direct patient care and coordinate care with other healthcare professionals to ensure comprehensive care for our veterans.
  • Assess, plan, implement, and evaluate care based on age-specific components and individual patient needs.
  • Administer medications and procedures per established policies and guidelines.
  • Collaborate with the interdisciplinary team to influence care outcomes and improve patient satisfaction.
  • Manage assigned patient panels and ensure timely access to care.
  • Provide health promotion, education, and coaching on wellness, disease prevention, and chronic care management.
  • Participate in the development, implementation, and evaluation of policies, procedures, and guidelines.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • At least one year of specialized nursing experience equivalent to the next lower grade level.
Preferred Qualifications
  • 1-2 years of experience in Primary Care, Outpatient, or Med-Surg.
