At Interim HealthCare Home Health and Hospice, we're seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. As a key member of our care team, you'll provide personalized home-based care to patients with illnesses, injuries, and chronic diseases.

Key Responsibilities:

  • Conduct thorough assessments and develop personalized care plans for patients
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families on medication management, therapy, and home safety
  • Monitor patient progress and make adjustments to care plans as needed
  • Ensure timely and effective discharge from services

Requirements:

  • Active, unrestricted RN license in the State of Michigan
  • At least one year of clinical experience as a Registered Nurse
  • Current CPR Certification
  • Reliable transportation with a valid state driver's license and current car insurance
