Job Summary

We are seeking a highly skilled Palliative Care Nurse Practitioner/Physician Assistant to join our team at Eventus WholeHealth. As a Primary Care Provider, you will deliver high-quality palliative care to patients in skilled nursing and assisted living facilities.

Key Responsibilities
  • Design and implement operational processes for palliative care programs
  • Work with facilities and Eventus employees to ensure the growth and success of palliative care programs
  • Train and educate internal employees on palliative care principles and processes
  • Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and standards of care
  • Manage narcotic prescriptions and ensure compliance with Eventus WholeHealth policies
  • Establish and maintain open and positive communications with facility staff and administration
Requirements
  • Masters degree in Nursing from an accredited educational institution and a current and valid license to practice medicine
  • Graduation from a physician assistant educational program accredited by a National Commission on Accreditation of Allied Education Programs
  • Specialization in geriatric, family, or psychiatry desired
