Job Summary

Community Rehabilitation Hospital North is seeking a skilled Licensed Practical Nurse to join our interdisciplinary team. As a Rehabilitation Nurse, you will play a vital role in providing high-quality patient care and contributing to our hospital's mission to maximize the health, function, and quality of life of those we serve.

Key Responsibilities
  • Assess and reassess patients to identify their individual needs and develop a plan of care that addresses their unique requirements.
  • Administer medications and treatments as prescribed by physicians and collaborate with the healthcare team to ensure seamless care.
  • Provide education and support to patients and their families to promote independence and self-care.
  • Participate in interdisciplinary rounds and contribute to the development of the patient's plan of care.
  • Collaborate with the healthcare team to ensure timely and accurate documentation of patient information.
Requirements
  • Graduate of an accredited Licensed Practical/Vocational Nurse program.
  • Current state licensure as a Licensed Practical/Vocational Nurse.
  • CPR/BCLS certification. ACLS preferred.
  • IV Certified required or obtained within 6 months highly preferred.
  • Minimum six months' Medical/Surgical experience in an acute care setting preferred.

Community Rehabilitation Hospital North is a 60-bed inpatient acute rehabilitation hospital operated jointly with Lifepoint Health and Community Health System. We are accredited by both The Joint Commission and CARF for our Stroke and Amputation Specialty Programs as well as CARF-accredited for our Comprehensive Integrated Inpatient Rehabilitation and Brain Injury Specialty Programs.
