Key Responsibilities
  1. Assess and evaluate residents' physical and mental health status, identifying areas for improvement and developing strategies to address these needs.
  2. Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each resident.
  3. Provide direct nursing care to residents, including medication administration, wound care, and other treatments as prescribed by physicians.
  4. Communicate effectively with residents, families, and other members of the healthcare team to ensure coordinated and comprehensive care.
  5. Develop and maintain accurate and up-to-date medical records, including charting resident progress, treatments, and any changes in condition.

Requirements
  • Education: Graduate from an accredited LPN program.
  • Licensure: Current and valid LPN license in the state of employment.
  • Experience: Prior experience in a senior living, long-term care, or similar healthcare setting is preferred but not required.
  • Skills: Strong clinical skills, excellent communication and interpersonal skills, ability to work effectively in a team, and proficiency in electronic health records (EHR) systems.
