Job Description

Overview:

North River Health Campus seeks a skilled and compassionate Licensed Practical Nurse (LPN) to serve as the Director of Health Services for our Assisted Living community. This leadership role requires strong clinical expertise, effective communication skills, and a passion for delivering exceptional care.

Responsibilities:

  • Directs the health concerns of each resident, including resident assessments, medication management, and personal care needs.
  • Hires and supervises nursing staff for the assisted living unit.
  • Ensures that services are performed in consideration of the health, welfare, safety, and satisfaction of our residents.
  • Collaborates with other directors on day-to-day operations of clinical services.
  • Conducts lifestyle and care needs assessments to determine initial and ongoing medical and personal care requirements.
  • Assists residents with activities of daily living, including bathing, dressing, grooming, dining, etc.
  • Maintains accurate and timely documentation in CareTracker.
  • Develops and communicates service plans to caregivers.
  • Serves as liaison between residents, families, and healthcare providers.
  • Coordinates pharmacy and medication needs, ensuring compliance with state regulations and company policies.
  • Supervises and participates in treatment regimens per physician orders.
  • Maintains resident medical records and chart information according to state guidelines.
  • Trains nursing assistants in activities of daily living assistance, pharmacy policies, and infection control.
  • Participates in hiring, recruitment, and training of clinical staff.
  • Ensures employee CPR/First Aid training meets state regulations and company policies.
  • Serves as weekend manager and provides on-call duties.
  • Organizes and directs marketing programs, building alliances with key referral sources.

Requirements:

  • Degree from an accredited school of nursing.
  • Current unencumbered LPN license in the state of practice.
  • Valid CPR certification.
  • Valid drivers' license and ability to operate a large van/bus vehicle.
