We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Axesspointe Community Health Center. Job Summary:

The Licensed Practical Nurse will be responsible for providing direct patient care, administering medications, and collaborating with healthcare professionals to ensure the best possible outcomes for our patients. This is a full-time position, working 40 hours per week, with a competitive salary range of $38,000 - $46,000 per year.

Key Responsibilities:
  1. Administer medications and treatments as prescribed by physicians and other healthcare professionals.
  2. Conduct clinical interviews and gather health information to inform patient care decisions.
  3. Collaborate with healthcare teams to develop and implement individualized care plans.
  4. Provide emergency care and first aid services as needed.
  5. Document patient information and maintain accurate records.
  6. Participate in staff meetings, in-service training, and other educational activities to stay current on best practices and policies.
  7. Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:

To be successful in this role, you will need:

  • A practical nursing degree from an accredited institution.
  • Current licensure as a Licensed Practical Nurse (LPN) in the State of Ohio.
  • Training in medication administration and first aid.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
