Home Health RN Case Manager

Amedisys Inc. is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing patient-centered care and collaborating with our interdisciplinary team to develop and implement individualized plans of care.
Responsibilities- Perform patient assessments and develop comprehensive plans of care in collaboration with the care team.
- Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
- Teach patients and their families self-care techniques and promote patient health and independence.
- Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
- Complete documentation timely and accurately, and communicate patient progress to the clinical manager and care team.
- Plan and provide staff education, and participate in clinical development and continuing education programs.
- One (1+) year of clinical experience as a Registered Nurse (RN).
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
