Job Summary
We are seeking a Primary Care Physician to join our team in Boston. As a Primary Care Physician, you will work in a concierge setting, collaborating with a nurse practitioner to provide high-quality care to our patients. You will have the opportunity to work across the entire continuum of care, including Day Center-based clinics, home visits, skilled nursing facilities, and hospital settings.
Key Responsibilities
- Act as the primary care physician for PACE participants in community and institutional settings.
- Provide routine care, episodic, and urgent care to patients.
- Perform initial and semiannual evaluations to ensure comprehensive care.
- Establish a collaborative practice with our PACE nurse practitioner to provide seamless care.
- Develop care plans for PACE participants in collaboration with our interdisciplinary team.
- Work collaboratively with members of our interdisciplinary team to ensure comprehensive care.
- Communicate with consulting physicians and integrate recommendations into plans of care.
- Prescribe medications in accordance with state and federal regulations.
- Participate in on-call rotation and attend interdisciplinary team meetings to ensure effective communication.
- Maintain accurate and up-to-date medical records and related documentation.
