We are seeking a skilled Clinical Care Specialist to join our team in the Transitional Care Unit at the University of Kentucky Chandler Hospital.

About the Role

This is a challenging and rewarding opportunity for an experienced Registered Nurse to provide exceptional patient care and collaborate with a multidisciplinary team to achieve optimal outcomes.

The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering compassionate care. As a member of our team, you will have the opportunity to work with patients across various age groups and acuity levels, from adult to geriatrics.

Key Responsibilities
  • Deliver high-quality patient care, prioritizing the needs of each individual and ensuring their safety and comfort.
  • Collaborate with physicians, nurses, and other healthcare professionals to develop comprehensive care plans and achieve positive patient outcomes.
  • Assist with decompressing overcrowded areas within the Emergency Department, Post Anesthesia Care Unit (PACU), and diagnostic/interventional radiology, as well as evaluating patients from outside facilities for appropriate level of care placement and direct admits from clinic, office, or home.
  • Provide education and training to Nursing Care Technicians (Nursing Assistants) and contribute to a positive and supportive team environment.
Requirements
  • Bachelor of Science in Nursing (BSN) degree required; AT LEAST 1 YEAR OF RN EXPERIENCE PREFERRED.
  • Current licensure as a Registered Nurse (RN) in the state of Kentucky.
  • Strong critical thinking skills and ability to work independently in a fast-paced environment.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
