Position Summary:

Hours: Full time, Monday - Friday, 8:30am - 5:00pm

Our nursing team practices a distinctive form of medicine recognized nationally for delivering continuous care across a spectrum of medically complex cases with multiple co-morbidities. We are seeking a dedicated and compassionate nursing professional for our Primary Care/Urgent Care Clinic to perform a comprehensive range of nursing duties, including direct patient care and nursing case management, immunizations, and screenings.

Key Responsibilities:

As a vital member of the interdisciplinary care team, you will participate in daily team huddles, clinic staff meetings, and breakout sessions. Your daily tasks will include educating patients about Patient-Centered Medical Home (PCMH) principles, conducting triage and nursing assessments for both walk-in and scheduled patients, and collaborating on care planning for high-risk patients. Additional responsibilities include:

  • Monitoring quality through record-keeping and analysis of patient health indicators.
  • Facilitating access to primary and episodic care for patients from outreach sites.
  • Coordinating patient care with BHCHP nurse liaisons.

Qualifications:

The ideal candidate will possess:

  • A valid RN license from the Commonwealth of Massachusetts.
  • Previous experience in a community health center or primary care setting.
  • Strong medical and surgical assessment skills.
  • A keen interest in public health and preventive nursing/medicine.
  • A commitment to serving underrepresented populations.
  • An understanding of substance abuse and mental health issues.
