Job Title: Fatherhood Coach-Case Manager

At Prevention Central, we are seeking a highly motivated and experienced Fatherhood Coach-Case Manager to join our team. As a key member of our staff, you will play a critical role in implementing fatherhood case management services and reviews for participants.

Job Summary:

The Fatherhood Coach-Case Manager will be responsible for:

  • Implementing case management services and reviews for participants
  • Collaborating with service providers to ensure comprehensive support
  • Planning and conducting face-to-face visits with fathers using evidence-based curriculum activities, methods, and materials
  • Supporting expecting or parenting fathers by facilitating educational, economic, employment, mental health, housing, and healthcare needs as required
  • Connecting fathers to needed resources and services in the community
  • Maintaining timely and accurate records of all visits, contacts, screenings, and resource linkages in our case management system
  • Maintaining participant confidentiality and integrity in keeping with agency policies, practices, and professional standards
  • Participating in outreach activities to identify fathers, inform the community of services, and build community collaborative relationships
  • Being available for evening and weekend activities as needed
Qualifications:

To be successful in this role, you will need:

  • An Associate's degree or a minimum of 3 years equivalent experience in community outreach, family support, or a related field
  • Experience in the social services field, preferably working with hard-to-reach, underserved, diverse populations
  • Experience facilitating groups and training
  • Experience working with dads is preferable
