About the Role

We are seeking a skilled and compassionate RN Hospice Care Specialist to join our team at SSM Health. As a key member of our hospice care team, you will play a vital role in providing high-quality patient care and support to individuals and their families in need.

Key Responsibilities
  • Assess and Coordinate Patient Care: Conduct thorough assessments of patients' medical and emotional needs, and coordinate care with the healthcare team to ensure comprehensive support.
  • Provide Skilled Nursing Care: Deliver skilled nursing care to patients in their homes, including wound care, IV administration, and other medical interventions as needed.
  • Collaborate with Interdisciplinary Team: Work closely with physicians, social workers, and other healthcare professionals to develop and implement individualized care plans.
  • Communicate Effectively: Maintain open and empathetic communication with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
Requirements
  • Education and Licensure: Graduate of an accredited nursing program and current RN licensure in the state of employment.
  • Experience: No experience required, but prior hospice or home health care experience is preferred.
  • Physical Demands: Ability to lift, bend, and stand for extended periods, as well as travel to patient homes and other locations as needed.
