Job Summary

We are seeking a highly skilled Social Worker to join our team at InnovAge. As a Social Worker, you will play a critical role in providing comprehensive care to our elderly participants, ensuring they receive the support and services they need to live independently.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify participants' needs and develop personalized care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Provide case management services, including referrals to community resources and support with housing and Medicaid eligibility.
  • Monitor participants' conditions and provide psychosocial support to address their unique needs.
  • Document all interactions and interventions in a timely and accurate manner.
Requirements
  • Master's degree in Social Work from a Council on Social Work Education-accredited program.
  • Current CPR certification or ability to obtain.
  • At least one year of experience in a healthcare setting working with the frail and elderly.
  • Strong case management and psychosocial assessment skills.
  • Personal transportation, valid driver's license, and good driving record.
Preferred Qualifications
  • Three or more years of social work experience.
