Responsibilities:

  • Assist Physical Therapists in the development and implementation of patient treatment plans
  • Conduct patient assessments and provide treatment as directed by the Physical Therapist
  • Maintain accurate and up-to-date patient records
  • Collaborate with the healthcare team to ensure seamless patient care
  • Participate in ongoing education and training to stay current with best practices
  • Ensure compliance with all relevant laws and regulations, including Covid-19 precautions
  • Verify and maintain accurate patient credentials, including state license verification and resume
  • Conduct background checks and reference checks as required
  • Ensure all necessary paperwork and documentation is completed and attached to the candidate
  • Review and ensure compliance with the credentialing checklist prior to submission
  • Participate in ongoing quality improvement initiatives
  • Perform other duties as assigned

Requirements:

  • Current state license as a Physical Therapist Assistant
  • SNF experience preferred, but new grads welcome
  • Covid-19 test 72 hours prior to start date with results issued 48 hours prior to start date
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Excellent communication and interpersonal skills
  • Ability to maintain confidentiality and handle sensitive information
  • Basic computer skills and proficiency in electronic medical records
  • Ability to work independently and as part of a team
  • Flexibility to work a variety of shifts, including days and evenings
  • Ability to lift, push, and pull equipment and supplies as needed
  • Ability to maintain a safe and healthy work environment
  • Ability to pass a background check and reference check
