Key Responsibilities- Assessment and Planning: Conduct thorough nursing assessments on admission and re-evaluate patient needs on succeeding visits to ensure optimal care.
- Care Coordination: Collaborate with the healthcare team to develop and implement patient care plans, including ordering and administering medications, treatments, and therapies as needed.
- Education and Support: Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle habits.
- Documentation and Communication: Maintain accurate and timely documentation of patient care, including progress notes, summaries of care conferences, and other relevant information.
- Case Management: Manage patient caseload, prioritize tasks, and ensure timely completion of assignments.
- Professional Development: Stay up-to-date with industry developments, best practices, and regulatory requirements to maintain expertise and provide high-quality patient care.
- Education: Graduate of an accredited school of nursing.
- License: Current RN licensure in good standing.
- Experience: One year of recent experience in home health nursing, preferably with Amedisys Home Health Care.
- Certifications: BLS certification and AHA certification.
