Mass General Brigham is seeking a skilled Physical Therapist to join our Home Hospital team in Chelsea, Massachusetts.
This dynamic role will involve providing acute-level care to patients in the comfort of their own homes, working collaboratively with a clinical team to identify patient needs and coordinate services.
Key Responsibilities- Develop and implement patient-centered plans of care, incorporating standardized interventions for goal-oriented outcomes.
- Conduct virtual and in-person patient evaluations, as needed, and document accurate and timely information to promote real-time updates for the clinical care team.
- Apply standard treatment procedures and provide applicable teaching to patients and caregivers.
- Reassess and modify the plan of treatment daily, as necessary, to meet discharge goals.
- Verify payor information and maintain documentation to support Managed Care authorizations and DME requirements.
- Registered with the Board of Registration in Massachusetts.
- Minimum of two years' experience as a licensed Physical Therapist, with homecare and/or acute care experience preferred.
- Strong customer service skills and ability to work with diverse populations.
- Excellent verbal, written, and electronic communication skills.
- Computer proficiency and ability to learn new software, with Epic and Rover experience preferred.
