Clinical Pharmacist Job Description
**About the Role**
We are seeking a highly skilled Clinical Pharmacist to join our team at UW Health. As a Clinical Pharmacist - Drug Policy Program, you will play a critical role in addressing drug shortage management, drug budget management, and formulary management.
**Responsibilities**
* Lead weekly drug shortage meetings and coordinate mitigation plans with clinical teams
* Implement cost savings targets based on available contracts and monitor utilization
* Develop, implement, and maintain clinical practice guidelines, drug use policies, and delegation protocols

**Requirements**
* Minimum - Bachelor's degree in Pharmacy or a PharmD degree
* Preferred - Completion of an ASHP Accredited Residency training program
* Minimum - One year of clinical patient care and orders management experience
* Preferred - Three years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program
* Certifications: Minimum - Licensed to practice pharmacy in the State of Wisconsin or must obtain within 90 days of hire date
