Job Title: Charge Nurse

Mass General Brigham is seeking a skilled Charge Nurse to join our Home Hospital team. As a Charge Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their homes.

Job Summary:

This is a 36-hour per week, day shift position that will service NWH, BWH, and BWFH. The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide skilled nursing care, including assessment, education, and treatment, to patients in their homes.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Manage patient schedules, adjust as needed, and communicate changes to the care team.
  • Reconcile medications accurately and timely, and document changes in the electronic health record.
  • Make referrals to other disciplines and community resources as needed to maximize patient safety and outcomes.
  • Attend staff meetings, competencies, and educational meetings to stay up-to-date on clinical initiatives and best practices.
Requirements:
  • Current Massachusetts license as a Registered Nurse.
  • Minimum of one year of recent nursing experience in an acute or emergency setting.
  • Home care experience a plus.
  • Ability to work with various computer software applications and technologies.
  • Strong interpersonal and customer service skills, with the ability to work with a diverse population.
