We are seeking a highly motivated and compassionate Care Coordinator to join our team at Chisholm Trail. As a key member of our senior care community, you will play a vital role in maintaining and monitoring our holistic wellness program, ensuring the highest quality of life for our residents.
Key Responsibilities:
- Assist the Director of Engagement with resident assessments and develop personalized care plans
- Lead neuroplasticity programming, including Connect-Move-Learn cycles, muscular strength, aerobic, and flexibility training
- Develop and implement social-recreational activities and outings, as well as volunteer program development and community resource coordination
- Collaborate with staff to create a supportive and engaging environment for residents
Requirements:
- High school diploma or equivalent
- Minimum 1 year experience in activities or recreation services
- Certified Dementia Practitioner or eligible for certification
- Strong interpersonal and communication skills, with a willingness to work in a hands-on role with residents and staff
- Ability to drive a company vehicle and meet approved driver requirements
