Job Title: Occupational Therapist Home Health

Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in Cuyahoga Falls, Ohio.

Job Summary:

We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in their homes. As an Occupational Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities:
  • Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
  • Conduct thorough evaluations to assess patient needs and develop individualized plans of care.
  • Implement and modify treatment plans to ensure patient progress and safety.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient progress and outcomes in accordance with agency policies and regulatory requirements.
Requirements:
  • Current licensure as an Occupational Therapist in the state of Ohio.
  • Minimum one year of experience as an Occupational Therapist in an acute care setting, with preference given to those with home care experience.
  • Valid driver's license and auto insurance.
  • Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
